The Fauquier Hospital Patient Portal is a secure, private web portal that allows you to access your health information online. Updating your personal information, checking on your visit history, account balances and your scheduled appointments is all available in one place.

Fauquier Health offers this portal to help you find all the information you need about your health information.
  • Make sure your healthcare provider has the correct contact information.
  • Update your health information, including allergies, medical conditions, medications you take and procedure history.
  • Review your visit history, insurance information, medications, reports and more.
  • Review your statements and pay your hospital bills online.
  • Check your appointment, directions and contact information, and which provider you’re seeing.
  • Verify your providers’ notes and nursing discharge information.
You can also use The Fauquier Hospital Patient Portal to keep track of your dependent’s information and share information you choose securely with people you trust, from your spouse to your sister or another provider.

You can archive your health record offline and save it in a convenient PDF file. You even have the option of providing an exchange of information to many of your providers’ electronic medical record systems.
What do I need to get started?
 
Just a few items:
  • Create a username
  • Create a password
  • Provide an e-mail address
  • Answer two security questions
  • Provide first and last name and date of birth (of the patient)
  • Enter the patient code (Given to you by your healthcare provider)
